A metabolic study of a patient with idiopathic cyclical thrombocytopenic purpura.

By WILLIAM A. SKOOG, JOHN S. LAWRENCE AND WILLIAM S. ADAMS C YCLICAL THROMBOCYTOPENIC PUHPURA related to menstrual periods has been the subject of infrequetst reports in the literature. Mitsot.’ in 1936 first described three cases of purpura hemssorrhagica associated with menstruation. He postulated that “sonse altered endocrine function” greatly exaggeratimig a drop of platelets thought to occur normally at the onset of menstruatiomi issight I)e responsible. No studies were performned to support this contentioms, however. The periodic episodes of thromssbocytopenic purpura ceased spontaneously after from three to seven attacks in these patients. Recently Pepper, Liebowitz, amid Lindsay2 reported another woman with this disorder. Their patiemst had an eight year history characterized by five attacks of localized purpura followinsg delivery and three episodes of gemseralized purpura of the skin associated with menstruatiomi. Repeated platelet counts demonstrated a fall in platelets to very low levels at the time of meisstruation with a rise to higher, but still abnormally low levels during the mid-cycle. Endocrinologic studies including urimsary 17-ketosteroids, 17-hydroxycorticosteroids and follicle stimulating hormotse levels done durimsg differeist phases of the menstrual cycle did not varv significatitly, amid thereby failed to iniplicate ams etidocrine factor. The presemst study concerns a nsiddle aged wonsams with an eight year history of intermitteist episodes of thrombocytopeisic purpura occurring cyclically in relatiots to her tnemistrual periods. Its comstrast to previous reports, however, our patient usually experienced purpura l)etween her periods of menstrual flow. It was considered that the unusual nature of this patient’s rare disorder warranted the extetisive metabolic research study described in this report.
